To obtain a Social Security Number:
There is no fee.
You will need to bring form SS5 (which can be found online at www.socialsecurity.gov) along with proof of citizenship, birth certificate and final adoption decree. All documents must be originals or certified copies by the custodian of the record. To find the location nearest you or if you have any additonal questions, visit their website or call 1800-772-1213.
